Student Health Insurance

Clark State Community College provides student health insurance through Sentry Life Insurance Company. The insurance agent, EJ Smith & Associates, is working through our local agent, Wallace & Turner, Inc. You can view the brochure and enroll online by clicking www.ejsmith.com. Click on Health Plans, select Ohio and then select 2008-2009.

The plan offers the following:

• Two basic plans (internal limits under basic plan; then major medical paid at 80%)
• Available to full-time and part-time students
• Coverage also available for spouse and children
• Coverage and premiums can be for a minimum of 4 months up to one year
• You can enroll anytime during the academic year
• No medical questions asked
• Pregnancy is covered (treated as any other sickness)
• Optional Discount Prescription Card
• Optional Dental Coverage
