Sports Physicals

Student athletes may obtain their sports physical in the Health Clinic at no charge. Interested athletes should contact their coach to arrange for an appointment with the clinic physician. Students choosing to obtain their physical from their family physician should use the Clark State Community College Sports Physical Examination form (PDF). These forms are also available from the Health Clinic, located in Rhodes Hall, Room 120. Completed forms should be returned to the Health Clinic and are kept on file for five years.
