MedicalClark State offers a select plan with United HealthCare of Ohio. The coverage is optional and provides for family, employee +1 or single coverage. There is an annual out-of-pocket deduction for prescription drugs of $100 for single coverage; $200 for employee + 1 and $300 for family coverage before the $10/$30/$50 co-pay is allowed. DentalOur dental plan is self-funded and administered by CoreSource. This benefit is fully paid by the College with no cost to the employee. Maximum benefit is $1,250 per calendar year. Life InsuranceClark State offers term life insurance with Kanawha Insurance Company. This benefit is fully paid by the College with no cost to the employee. Additional supplemental term life insurance with coverage ranging from $10,000-$40,000 is available via payroll deduction. The basic life insurance benefit is 1.5 times the annual base salary for all eligible employees. VisionOur vision carrier is VSP. Single coverage is fully paid by the College. Employee +1 coverage is available at a cost of $3.04 per month; family coverage is available at a cost of $10.04 per month on a tax-sheltered basis via payroll deduction. HearingClark State has recently established a PHCP (Preferred Hearing Care Provider) relationship with Avada Audiology & Hearing Care. This program entitles you and your family to receive discounts on hearing instruments and related products/services at Avada locations throughout Western Ohio. Product discounts include 1% on all HearO and PHD hearing instruments and 5% discount on all Aura hearing instruments. Cancer, Disabilty and Universal Whole LifeClark State offers these optional coverages with Colonial Life Insurance through payroll deduction. To determine the cost and coverage of these insurances, please contact our Colonial representative. CobraUnder federal law, eligible employees may continue health benefits if there is a qualifying event (e.g. termination/resignation). Benefits can continue for 18 months (employee) or 36 months (spouse and dependents) after the date of the qualifying event and if premium payments are made by the (former) employee in accordance with the law. Retirement PlansSchool Employees Retirement System of Ohio (Defined Benefit Plan) - Non-teaching positions: 10% of the employee's gross pay is withheld on a tax-deferred basis. The College contributes 14% of the employee's gross pay. State Teachers Retirement System (Defined Benefit Plan) - Teaching positions: 10% of the employee's gross pay is withheld on a tax-deferred basis. The College contributes 14% of the employee's gross pay. Alternative Retirement Plan (Defined Contribution Plan) - This plan is for eligible employees with five years of service or less. This retirement plan is portable. 10% (non-teaching positions) and 10% (teaching positions) of the employee's gross pay is withheld on a tax-deferred basis. The employee can choose from several different providers. The College contributes between 10%-14% of the employee's gross pay. 403(b) Tax Deferred Annuity (Defined Contribution Plan) - Employees can contribute additional monies for retirement on a tax-deferred basis. The College does not match this contribution. FICA - Mandatory participation for employees hired after April 1986. 1.45% of the employee's gross pay is withheld on a tax-sheltered basis. The College contributes 1.45% of the employee's gross pay. Flexible Spending AccountThe Flexible Spending Account is a tax free benefit that may be used for health care reimbursement (medical, vision, and dental expenses) as well as dependent day care reimbursement. This benefit is deducted from each bi-weekly paycheck and begins upon employment or beginning of plan year, September 1. LeaveVacation - Eligible exempt and non-exempt employees accrue vacation on a bi-weekly basis based on grade level of position. Vacation is prorated for part-time employees. Maximum accrual of 20 days allowed. Sick leave - Eligible employees accrue sick leave on a bi-weekly basis. Sick leave is cumulative with no maximum limit. Employees accrue 1.25 days per month (4.6 hours bi-weekly; 15 days annually). Sick leave is prorated for part-time employees. Personal Leave - Eligible employees are granted two days each fiscal year. Personal leave cannot be carried forward from one fiscal year to the next and must be taken in either 1/2 day or full day increments. Personal leave is prorated for part-time employees. Holidays - The College observes ten legal holidays. One additional "floating holiday" is provided to eligible exempt and non-exempt employees. Please refer to the official College calendar. Professional Improvement Leave (Sabbatical) - Leave may be granted to exempt employees and instructional faculty. This provides an opportunity for improved service to the College through study, research, professional writing or travel abroad. If leave is for three quarters, the employee is eligible to receive 50% of their salary. If leave is for two quarters, the employee is eligible to receive 66% of their salary. Family Medical Leave Act (FMLA) - Eligible employees may be granted up to 12 weeks leave during a 12-month period. To qualify for this leave, guidelines under the FMLA must be met. Personnel DevelopmentTuition Reimbursement - After one year, eligible employees may be reimbursed up to nine credit hours (per fiscal year) for approved courses taken at another institution. Undergraduate reimbursement is up to $166 per credit hour or $249 per semester hour. Graduate reimbursement is up to $225 per credit hour or $337.50 per semester hour. Institutional Fee Benefit - Eligible employees and their families (spouse and children) may take credit and noncredit classes at the College. The benefit covers instructional and general fees. Some exceptions apply to noncredit courses. PrivilegesDiscounts - College employees receive discounts from Verizon (15% discount for wireless service), the Clark State Bookstore (10% off non-sale price), Performing Arts Center and a variety of restaurants. Computer Loans - Eligible employees may request an interest-free loan of up to $2,500 to purchase a personal computer for home. Library, Wellness Center, and Health Clinic - Employees can enjoy free access to all of these facilities. Checking out books, exercising on state-of-the-art equipment and having your blood pressure checked are just a sampling of services available by utilizing these facilities. Parking - The College provides parking to employees at no cost.
