The Records and Registration Office processes your student records, transcripts, grades, ID cards and diplomas, as well as your class registration each semester.

How to Request a Transcript
Clark State is pleased to offer secure online transcript ordering through the National Student Clearinghouse (NSC), a non-profit organization serving higher education. There is a $2 per transcript charge no matter how you place your order. There is an additional $2.25 fee for each location that a transcript is sent when ordering online.
You can also request a transcript in person in the Records Office in Rhodes Hall on the Springfield Leffel Lane Campus or at the Beavercreek Campus during normal operating hours. Transcripts typically take 4-5 business days to process when ordered in person.
We strongly encourage you to order your transcripts online. If you are unable to do so, you may send a written letter of request to: Records Office, Clark State Community College, PO Box 570, Springfield, OH 45501. Written transcript requests may take 5-10 business days to process once the request is received.
Written requests must contain all of the following information: Complete current name, previous name(s) used while attending Clark State, Social Security Number or student identification number, date of birth, approximate years of attendance, a contact phone number where you can be reached if there is a problem with your order, number of transcripts requested, name(s) and address(es) where the transcript is to be sent and your signature authorizing release of your transcript.
Clark State accepts cash, check, money order, VISA, MasterCard and Discover Card. Payment must accompany the transcript request.
Transcript requests will not be accepted by fax or e-mail. Students with past due financial obligations to Clark State may have their transcript orders suspended or returned. To resolve outstanding financial obligations, contact the Cashier's Office at 937-328-6048.
Update Your Information
If you would like to change your address, major or telephone number, please complete a Change of Information card online.
If you need to change your name, Social Security Number or date of birth, please download the Record Change Request form on the myClarkState portal and return it to Records and Registration or the Greene Center with the requested documentation.
You may also fax your request to us at 937/328-6097 or make your request in person at the Records and Registration Office. Your written request must include specific information. Please call the Records and Registration Office for details.
