Please don't hesitate to do so. We take the safety of our students very seriously and will not tolerate any of the below incidents.
Clark State Community College separates the College’s incident reporting structure into two categories listed below. If you have questions or concerns about which report to use, please contact the Dean of Student Engagement and Support Services in the Sara T. Landess Technology and Learning Center, Room 115 or call 937.328.7936.
Stalking, Sexual Misconduct and Relationship Violence
Reporting Sexual Misconduct
The Sexual Misconduct Form may be used to report information to the Clark State Community College Title IX Office related to an alleged violation of the Clark State Community College Sexual Misconduct Policy by a member of the Clark State community or a visitor to Clark State. The Clark State Sexual Misconduct Policy prohibits sexual misconduct of various forms, including but not limited to: sexual harassment, sexual violence, gender-based harassment, intimate partner violence, stalking, intimidation and retaliation.

Violation of Student Code of Conduct or General Incident on Campus
Reporting Behavioral Issues or General Incidents
Clark State Community College is concerned with the safety, health and well-being of everyone on campus. Faculty, staff, students and guests are encouraged to use the Online Referral Form to report an incident that involves a behavioral issue, improper conduct, threatening behavior or is a violation of the student code of conduct. The form can also be used to report a general incident, such as an accident on campus
