Your safety and well-being matter at Clark State College. If you see something concerning, experience an issue or need to report an incident, please let us know. Reporting helps us respond quickly and maintain a safe, respectful campus community.

Report Behavioral Issues or General Incidents

Faculty, staff, student and guests may use the Incident Reporting Form to report concerns involving safety, behavior, accidents or general incidents occurring at any Clark State location.

Report Sexual Harassment, Discrimination or Title IX Concerns

Clark State is committed to maintaining an environment free from sex discrimination. Complete the Sex Discrimination Form to report concerns involving employees, students, or third parties, including sexual harassment, sexual misconduct, sexual assault, dating or domestic violence, stalking or any other form of sex discrimination. All reports are reviewed by the College’s Title IX Office in accordance with institutional policies and procedures.

Academic Concerns

If you have an academic concern (grading, classroom experience, instructional issues), first attempt to resolve the issue directly with your instructor. If the issue is not resolved, submit the Academic Concern Form.

CARE Team Referral

The CARE (Concern, Assessment, Response, and Evaluation) Team supports students who may be struggling with academic, personal or emotional difficulties or exhibiting concerning behavior. Faculty, staff, students and others may refer a student. This is a non-emergency reporting option. If there is an immediate threat of harm, call 911.

Office of Student Support Referral

Refer a student who may be facing barriers affecting their academic or personal success. Students are encouraged to self-refer. This is a non-emergency reporting option. If there is an immediate threat of harm, call 911.

Student Complaint Information (State-Level Reporting)

The Ohio Department of Higher Education is responsible for responding to formal complaints against public, independent non-profit and proprietary institutions of higher education in Ohio. While the ODHE has limited authority over colleges and universities, and cannot offer legal advice or initiate civil court cases, the Chancellor's staff will review submitted complaints and work with student complainants and institutions.
